Obtaining Medical Records
Written authorization is required for medical records from Duke University Hospital, and must be submitted directly to the hospital’s Health Information Management department.
There are three walk-in locations at which you can visit the Medical Records Department and obtain your medical records. If you are coming in person to request records, please make sure you have one of the following forms of ID: driver’s license, passport, DMV ID card, military ID card, or government issued ID. Our ability to provide same day service on site will be dependent on several factors:
- Availability of records
- Volume and age of records requested
- Requested format (for example: flash drive, paper, or CD)
- Additional approvals needed
You may pick up your medical records in the Health Information Management Department during normal business hours (see hours and addresses below). We are also available by phone at 919-384-7119 to answer any questions you may have on completing the release form or any general release of information questions.
