Insurance and Payments
We ask that you bring one form of identification, such as a driver’s license, insurance cards, and authorization forms. We will process claims for services with your insurance carriers. At the time of your visit, you may be asked to pay amounts not covered by your insurance, such as co-pays, deductibles, and coinsurance. If you have multiple appointments, your insurance company may require multiple co-payments.

Duke is committed to providing care with dignity and respect. If you have questions or cannot pay your bill in full, please let us know so we can help. Our patient service associates, financial care counselors, and patient financial coordinators will help you fill out applications for monthly payment plans, financial assistance programs, and government-sponsored programs.
